We welcome New Patients to the Lifelong Health Family! We are excited to meet you, and it is our passion to get to the root of your problem and help.
Please arrive about 10 minutes early to your appointment so we can give you a tour of the office and process all necessary paperwork. To save time in the office, please fill out your intake forms, linked below, in advance. If you have insurance that you'd like to contribute to care, please bring your card along. No special attire is required. Plan to be in the office for less than an hour.
